Behavioral Health Emergency Services

The Behavioral Health Emergency Services Program provides services to residents and consultation to professionals in Polk County, Wisconsin, 24 hours a day, 7 days a week through  a contract with Northwest Connections by dialing 911 and asking for the on call Behavioral Health worker, or calling 1.888.552.6642.

Emergency Services Include:

  • Mobile on-site response to assess and stabilize a crisis situation
  • Short-term stabilization services
  • Telephone crisis intervention
  • Mental Health consultation
  • Mental Health crisis consultation to community providers and law enforcement
  • Assessment of risk to self or others
  • Individual  and family crisis assessments interventions
  • Coordination of out-of-home placements including psychiatric hospitalization, if necessary
  • Alcohol and Other Drug Abuse Services

These services are provided in homes, police departments, hospitals, schools and other community locations.

Civil Commitments

Civil Commitment is a process in which the state deprives an individual of his/her personal freedom and ability to make treatment decisions in order to protect that person and/or others from harm.

  • 3-party petitions for involuntary commitments. Staff assistance is available.

Non-Emergency Services Include:

Targeted Case Management

Targeted Case Management (TCM) is an information, referral and resource program which serves adults with chronic mental illness. These services are time limited to the needs; not long-term/ongoing services (1 year on average).


Services Provided:

  • In-depth assessment of strengths and needs
  • Development of an individualized plan of care
  • Ongoing monitoring
  • Service coordination
  • Appropriate recommendations and referrals

Outreach is a time-limited service, intended to identify and contact chronic mentally ill residents of Polk County, Wisconsin, that may be in need of mental health assessment and/or support services. Behavioral Health Outreach provides mental health intervention which includes: information, education of available Behavioral Health Services, assessment of symptom status, crisis stabilization, recommendations and referrals. Individuals referred to this service will be afforded the opportunity to be assessed in an environment that provides comfort and safety for the individual and county staff which may include the individual's home, a county office site or neutral setting.
