Polk County Behavioral Health Department provides out-patient services in both mental health and substance abuse services. If you or someone you know is experiencing an emergency mental health crisis, call the 24 Hour Mental Health and AODA Crisis Line at 1-888-552-6642 or 911.

Referral Process

Please call 715.485.8400 or email to request services or to set up an appointment.


We will not deny services based on a person's race, color, sex, national origin, political beliefs, disability, sexual orientation, age or religion. Priority will be given to pregnant women in need of substance abuse services.


We offer discounted fees for patients who qualify. Our office accepts most major insurances including Medicaid. See our current Service Fee Schedule in the PDF below.
